The Cambridge-North Dumfries Ontario Health Team (CND OHT) comprises 32 agencies and health services providers from Cambridge, North Dumfries and the Waterloo Region.
Our purpose is to reorganize and deliver health care in a different way for our communities. The following partner and associate agencies will work together as one coordinated team to deliver primary care, rehabilitative, hospital care, long-term care, mental heath & addictions services and more.
