Patients connect with the clinic online, by phone, or in person. We confirm eligibility and collect initial information.
2. Initial Appointment
A clinical visit to review health concerns, history, medications, and preventive care needs.
3. Short-Term Care and Follow-Up
Ongoing support may be provided, including follow-up visits, referrals, and coordination of care.
4. Transition to Ongoing Care
We support a smooth handoff to a permanent family doctor or nurse practititioner.
