The purpose of the Cambridge and North Dumfries Ontario Health Team (CND OHT) Patient, Family and Caregiver Advisory Council is to engage with patients, families, and caregivers to increase access to care, improve care coordination and collaboration, and improve health outcomes and experiences for everyone accessing CND OHT services.
The Cambridge North Dumfries Ontario Health Team (CND OHT) is working to improve the wellbeing of our communities. This is one way that the CND OHT will connect with local communities.
