A Collaborative Quality Improvement Plan (cQIP) is a population health management improvement plan designed to align provincial and local health system priorities with the Quintuple Aim—reducing costs, enhancing population health, improving patient and provider experiences, and promoting equity. It focuses on addressing the needs of populations at higher risk. The plan outlines the approach an OHT will take throughout the year to apply an equity lens systematically, identifying and addressing care gaps using quality improvement and change management principles.
The CND OHT cQIP work is lead by the Quality Co-Design group, with engagement from all members of our OHT.
