2024-2025 CND OHT Preventative Frailty Screening Project

 

In collaboration with members of the CND OHT Older Adult Work Stream, the CND OHT is currently piloting a Preventative Frailty Screening Project. Frailty is a condition of reduced function and health commonly seen in older adults, that can result in vulnerability to sudden or dramatic changes in health due to seemingly small events. The project will support early identification and intervention of frailty, to equip older adults with the support they may need to age well and at home in the community.
 
If you have received an email from your Family Doctor inviting you to complete this brief assessment as part of a plan to keep you healthy, please see below for more information:

 

Why have I received this email and am I required to complete it?

  • Your Family Doctor is participating in the project and may be screening all older adults in their care over a certain age. Your participation is voluntary.
  • Even if you are feeling well, it is helpful to complete the assessment, so your Family Doctor is updated and able to ensure you are equipped with tips to help you stay strong and healthy as you age.

 

How do I take the assessment?

  • The link will take you to an assessment, that should take no more than 10 minutes to complete.
  • The assessment will ask you a series of questions related to activities of daily living, falls, and nutrition. Please answer as honestly as you can.

 

What happens after I take the assessment?

  • You will be linked to the Healthy Aging Resource Portal, where you can find resources to support your healthy aging.
  • Your Family Doctor will receive your assessment and review the responses.
  • You may receive follow-up communication from your Family Doctor, a local Health Guide, or a Dietitian, to discuss next steps to support healthy aging.

  • Rest assured, this assessment is designed to identify your risk of frailty and connect you with access to services or resources as required, to support you with healthy aging.

 

 

CND OHT Older Adult Healthy Aging and Frailty Management Integrated Care Pathway
The CND OHT Older Adult Work Stream, under the clinical leadership of Dr. Tamara Holling, has established an integrated care pathway for older adults, their care partners, and their provider to ensure that they receive the care that they need, when they need it. This may include:

 

  • Early intervention to prevent frailty
  • Rapid and streamlined referrals to manage early frailty
  • Wrap-around support to manage complex frailty
