Waterloo Region Front Line Navigators Community of Practice
The Cambridge North Dumries Ontario Heath Team (CND OHT has partnered with the KW4 Ontario Health Team (KW4 OHT) to improve system and patient navigation across the Waterloo Region. This collaboration aims to support health and social service navigators in helping patients find services they need, when they need them, by enhancing overall accessibility and coordination of care.
CND OHT supports this work in the following ways:
- Creating a community of practice with bi monthly meetings, provides the navigators with a dedicated space to build relationships, share ideas and resources, and learn from each other.
- Establishing a secure digital messaging space to enable navigators to further connect outside of the regular bi-monthly meetings.
- Collaborating with regional partners to align website resources and communication strategies across organizations to support this initiative.
