Ontario Health Teams will help to transform the provincial health care landscape. By building high-performing integrated care delivery systems across Ontario that provide seamless, fully coordinated care for patients, Ontario Health Teams will help achieve better outcomes for patients, improved population health, and better value for the province.

 

The Cambridge-North Dumfries Ontario Health Team (CND OHT) comprises 40+ agencies and health services providers from Cambridge, North Dumfries and the Waterloo Region.

  • Put patients, families and caregivers at the centre of a re-designed health care experience.
  • Improve collaboration, care coordination and knowledge sharing across all partners.
  • Reduce barriers such as discrimination, stigma, culture and language to improve access to care.
