CND OHT Became a Best Practice Spotlight Organization (BPSO)

Working with the Registered Nurses Association of Ontario (RNAO), CND OHT became a Best Practice Spotlight Organization (BPSO)! This work launched on November 22 with 32 attendees representing organizations across Cambridge and North Dumfries.

A BPSO OHT is an OHT that partners directly with RNAO to implement best practice guidelines (BPG) in order to better respond to health system transformation in Ontario.

Community Mental Health and Addictions Clinic (C-MAC)

From March-April 2023, the Cambridge North Dumfries Ontario Health Team (CND OHT) piloted an eight-week Community Mental Health and Addictions Clinic (C-MAC). The C-MAC was established to improve quick access to mental health and addictions treatment, improve the patient experience in navigating treatment and alleviate system pressures.

The C-MAC was initiated by the CND OHT Mental Health and Addictions Work Stream, under the clinical leadership of Dr. Craig Albrecht. The pilot was operationalized by several CND OHT Members and community partners, including:

• Waterloo Region Nurse Practitioner-Led Clinic
• Canadian Mental Health Association – Waterloo Wellington
• Two Rivers Family Health Team
• Porchlight Counselling and Addictions Services
• Grandview Medical Centre Family Health Team
• House of Friendship
• Region of Waterloo Paramedic Services
• Region of Waterloo Public Health​

Project Status

The C-MAC pilot is now complete. The CND OHT is working to secure sustainability funding to re-launch C-MAC services for Cambridge and North Dumfries.

Older Adult Healthy Aging and Frailty Management Integrated Care Pathway

About

The CND OHT Older Adult Work Stream, under the clinical leadership of Dr. Tamara Holling, has established an integrated care pathway  for older adults, their care partners and their provider to ensure that they receive the care that they need, when they need it. This may include:

• Early intervention to prevent frailty
• Rapid and streamlined referrals to manage early frailty
• Wrap-around support to manage complex frailty
