The Cambridge North Dumfries Ontario Health Team has several population health management and health system improvement projects underway. This project page will be updated regularly to share status updates and evaluation results.
From March-April 2023, the Cambridge North Dumfries Ontario Health Team (CND OHT) piloted an eight-week Community Mental Health and Addictions Clinic (C-MAC). The C-MAC was established to improve quick access to mental health and addictions treatment, improve the patient experience in navigating treatment and alleviate system pressures.

The C-MAC pilot is now complete. The CND OHT is working to secure sustainability funding to re-launch C-MAC services for Cambridge and North Dumfries.
