On September 29, 2026, the Ministry of Health, in collaboration with Ontario Health, launched the 2027–2028 Interprofessional Primary Care Team (IPCT) Expansion Call for Proposals (Round 3).
This funding opportunity represents an investment of more than $250 million to support the attachment of an additional 600,000 people across Ontario to primary care.
Building on the first two rounds of investment, round 3 aims to expand access to comprehensive, team-based primary care, particularly for individuals who do not currently have a Family Physician or Nurse Practitioner.

The Cambridge North Dumfries Ontario Health Team (CND OHT), in collaboration with the Cambridge North Dumfries Primary Care Network (PCN), is coordinating a community-wide approach to the IPCT 3 application process.
Our goal is to develop a collaborative, evidence-informed application that reflects the needs of our community, strengthens existing primary care services, and expands access to team-based care for residents of Cambridge and North Dumfries.
To support this approach, CND OHT is inviting local primary care providers and organizations to submit an Expression of Interest (EOI) outlining their proposed initiatives and opportunities to expand access to interprofessional primary care services.
Rather than treating this process solely as a competition between individual organizations, CND OHT aims to identify opportunities for collaboration across the local primary care system.
Submitted EOIs will be reviewed through a structured evaluation process. Based on the results, organizations may be invited to work together to refine or combine elements of their proposals into a coordinated application.

Submitting an EOI does not guarantee that a proposed initiative will be included in the final application or receive funding. All proposals will be assessed against the provincial requirements and local community priorities.
CND OHT welcomes Expressions of Interest from eligible primary care providers and organizations interested in expanding access to team-based primary care within Cambridge and North Dumfries.
Step 1: Review the Funding Opportunity
Interested organizations are encouraged to review Ontario Health’s Round 3 Call for Proposals instructions and supporting documentation.
Applicants should consider how their proposed initiatives contribute to the four provincial priorities, particularly the attachment of individuals who do not currently have a primary care provider.
The majority of the CND unattached population is in the following Forward Sortation Areas (Postal Codes):
N1R : 4413 individuals
N3H: 2708 individuals
N3C: 2323 individuals

Step 2: Submit an Expression of Interest
CND OHT will begin accepting Expressions of Interest on October 15, 2026.
Interested organizations will be asked to provide information about their proposed initiative, anticipated impact on primary care attachment, implementation readiness, and opportunities for collaboration.

Step 3: Proposal Review and Evaluation
Following the October 28 deadline, submitted EOIs will undergo an eligibility screening and structured evaluation process.
An evaluation committee, including representation from patients and families, will review eligible proposals and develop recommendations.
The review will consider alignment with Ontario Health’s funding priorities, local population health needs, implementation feasibility, and opportunities to strengthen primary care services across Cambridge and North Dumfries.
Step 4: Collaborative Application Development
Following the evaluation process, recommendations will be presented to PCN leadership.
Selected applicants may be invited to participate in further discussions to refine their proposals, explore opportunities for collaboration, or contribute to a coordinated community application.
The final application will be developed in consultation with relevant partners and reviewed through CND OHT’s leadership and endorsement process before submission to Ontario Health
Key Dates and Application Timeline
CND OHT has established the following timeline to support a coordinated and transparent application process.
October 28, 2026 — Local Expression of Interest Deadline
All organizations interested in participating in CND OHT’s coordinated application process are encouraged to submit their EOI by this date.
November 20, 2026 — Provincial Application Deadline
This is the official deadline for submitting completed applications to Ontario Health. CND OHT is targeting November 18 to allow time for final review and submission.
